Use a clear, complete prescription issued by your optometrist.
Copy each value exactly as it appears on your prescription. Leave CYL and AXIS blank only when they are not provided.
OD means right eye and OS means left eye. SPH is lens power, CYL and AXIS describe astigmatism, ADD is used for reading power, and PD is the distance between your pupils.
